How can we do this?

We practice medicine using a Direct Primary Care model. Direct Primary Care is a membership-based model where patients pay a reasonable monthly fee and, in return, get direct, personal access to their physician; virtually unlimited appointments; significantly discounted cash-based labs (we frequently beat insurance-based costs); and so much more.

However, if you cannot, we will go over this with you on your first visit.  You will receive forms in your e-mail (bear with me, I know this is a lot but once done, you should not need to do most of these again): Direct Primary Care Patient Agreement packet If you receive Medicare benefits, you will receive an additional Medicare private contract form Release of Information Form (You may need to do this for each provider you see. We can discuss this further.) Intake/Medical history form  You will also receive an access link from Hint which will allow you to manage your membership and payment information. Book your first appointment. This is a 90-minute appointment to establish care. We will review your history and create a foundation for your health plan. Please respond to this e-mail if you’d like this appointment to be virtual and I can arrange that for you. If these times do not work for you, please respond to this e-mail or call with your availability.  Please note, we are unable to provide care until all the agreements are completed.
